The Tertiary Transition Functional Profile is for anybody, whether they are a secondary school student planning life after school or a mature student looking to return to study.

It is designed for people who want clearer information about career pathway fit, study participation and sustainability, and readiness for study before committing significant time, money, and energy to a pathway.

The report provides practical information that clients and relevant stakeholders, such as parents, school guidance counsellors, tertiary support staff, and treating clinicians, can use to inform tertiary transition planning.

Where disability-related adaptive functioning is relevant, a disability-aware assessment pathway may be used.

A disability-aware version of the Tertiary Transition Functional Profile is available where disability-related adaptive functioning is relevant to tertiary study or training planning.

The appropriate assessment pathway is determined from the referral information, supporting documentation, and the Occupational Therapist’s clinical judgement.

The Tertiary Transition Functional Profile costs AUD $1,255, whether accessed privately or through NDIS funding.

The fee includes the required assessments and questionnaires, guided assessment instructions, review and interpretation of the completed assessment information by an AHPRA-registered Occupational Therapist, synthesis of the findings, and preparation of the final report.

The Tertiary Transition Functional Profile may be considered for NDIS funding where it is relevant to the participant’s disability-related needs and a relevant employment, work, or study-related NDIS goal.

Potential claiming pathways may include:

• Finding and Keeping a Job - Employment Related Assessment, Counselling and Advice (Occupational Therapist), where the assessment is connected to a relevant employment or work pathway goal.

• Daily Assistance Capacity Building - Assessment, Recommendation, Therapy or Training (Occupational Therapist), where appropriate to the participant’s plan and circumstances.

The participant-specific funding position should be confirmed with the participant’s Plan Manager and/or Local Area Coordinator before proceeding.

If the participant does not currently have a relevant employment, work, or study-related NDIS goal, or requires supporting documentation to seek funding for the Tertiary Transition Functional Profile, Tertiary Transition may be able to provide a support letter where clinically appropriate. Information relevant to the support letter can be provided through the referral form.

Tertiary Transition is not a registered NDIS provider and currently accepts NDIS-funded referrals from self-managed and plan-managed participants only.

1. Complete a referral form.
   Provide the client’s details and relevant background information.

2. Receive confirmation.
   Clients receive a Service Agreement and Consent Form to sign.

3. Receive the assessment instructions.
   Once the Service Agreement and Consent Form are signed and the service is confirmed, assessment instructions and links are emailed to the nominated contact.

4. Complete the assessments and questionnaires.
   Complete the required assessments and questionnaires using the emailed instructions.

5. Clinical interpretation and report preparation.
   The completed assessment information is reviewed, interpreted, and synthesised into the report by an AHPRA-registered Occupational Therapist.

6. Report delivery.
   The final report is issued electronically to the nominated email address.

7. Follow-up questions.
   Written email follow-up is available for 14 days after the report is issued.

It identifies participation patterns, potential pressure points, and practical conditions or strategies for consideration or trial.

The Tertiary Transition Functional Profile does not provide a diagnosis and is not therapy or ongoing treatment. It is not a Functional Capacity and Support Needs Assessment and does not recommend specific courses, education providers, careers, funded support amounts, support worker hours, supervision levels, or personal care requirements.

The service is available worldwide.

Reports are written in English and prepared by an Australian-registered Occupational Therapist.
