The Tertiary Transition Functional Profile is non-diagnostic and non-therapeutic. It does not replace medical or psychological assessment and does not recommend specific courses or education providers.

1. Complete a brief intake form here.
   Start by filling out a simple online form to share your details and goals.

2. We call you to arrange an online appointment.
   You’ll receive a phone call to book an online appointment time that works for you and your family.

3. Attend your online appointment.
   Meet with the occupational therapist for a personalised assessment and discussion.

4. Report preparation.
   The occupational therapist reviews the assessment findings and writes a personalised, professional report for you.

5. Report delivery.
   Your report is sent to your nominated email with secure access.

6. Optional follow-up.
   You can request written email follow-up for clarification if needed.

• A laptop or desktop with camera and microphone is recommended.

• If using a mobile phone or tablet, the Microsoft Teams app must be installed (browser access is not supported on mobile).

• Video and audio are required; phone call only appointments are not suitable.

Appointments are available Monday to Friday, and on weekends by arrangement to accommodate busy family schedules (no additional cost).

The Tertiary Transition Functional Profile is AUD $1,930.00 incl. tax.

This is the total fee and includes assessment costs, an appointment with an experienced occupational therapist, clinical interpretation, report writing, and follow-up after the report is completed.
