Decisions around tertiary transition deserve the same level of informed planning we apply to other major life decisions.

Let us help you navigate the transition with clarity.

The Tertiary Transition Functional Profile™ is a comprehensive functional guide designed to support personalised, informed tertiary transition planning and preparedness.

This isn’t just career direction. The assessment is led by an experienced occupational therapist and applies a clinical lens to examine how well different tertiary pathways may fit the individual, what factors need to be considered to manage study demands and support sustainability, and how prepared someone is for the day‑to‑day realities of tertiary study.

The Tertiary Transition Functional Profile is structured around three pillars:

  • Pillar 1: Career Pathway Fit

Clarifies the kinds of work activities, learning demands, and environments most likely to remain engaging over time, and where predictable friction is likely to emerge so it can be planned for early.

  • Pillar 2: Capacity and Sustainability for Tertiary Study

Considers the demands of tertiary study and how these interact with the person’s capacity to manage study load, particularly as external pressures increase.

  • Pillar 3: Independent Living Readiness

Looks at the practical life factors that influence stability while studying, including readiness to independently manage everyday responsibilities alongside study, such as routines, organisation, finances, and personal administration.

  • Decisions about pathway choice, timing, workload, and expectations can have significant academic, financial, and life implications.

The Tertiary Transition Functional Profile is particularly useful for young people transitioning from secondary school into tertiary education, and for anyone considering tertiary study or training, including as part of a career change, who is seeking clearer insight into career pathway fit and what study participation is likely to look like in practice.

It can also help align planning with key stakeholders such as parents or carers, university or TAFE support staff, and treating clinicians.

The Occupational Therapist behind the Tertiary Transition Functional Profile.

Occupational Therapist Tom Underhay

Tom Underhay (Bachelor of Occupational Therapy) | Australian Health Practitioner Regulation Agency (AHPRA) Registered Occupational Therapist.

My vision for the Tertiary Transition Functional Profile.

My goal is simple: to give clients a useful report that helps them make informed decisions and plan ahead with confidence, because tertiary transition decisions deserve the same level of informed planning we apply to other major life decisions.

This is personal for me. Earlier in my own tertiary journey, I chose a pathway that looked right on paper and I performed well academically, but the work environment was a mismatch. Over time, the friction added up and I stepped away from that direction. Looking back, a practical functional profile would have clarified career pathway fit and likely pressure points before I committed significant time, money, and energy.

I keep this service intentionally small and clinician-led, so clients have direct contact with an experienced clinician throughout the process.

My clinical experience.

I have 10+ years of experience as an Occupational Therapist across Australia and New Zealand, including acute care, neurological rehabilitation, community services, the NDIS, hand therapy, and aged care. This breadth underpins an occupation-focused approach that translates real-world demands into practical planning guidance for tertiary transition.
